Print this form and mail it with your check to the CPC treasurer: Reed Gleason 9615 NW Skyline Blvd Portland OR 97231-2614 Annual dues, payable in December, are $30 for individuals, or $38 for families living at the same address. If it's not December when you are joining or renewing, please pay a prorated amount of $2.50/mo or $3.17/mo for the number of months remaining in the year. You could also add the $30 or $38 to cover the next year as well.
